How to Submit Medication Refill Request ?
To request a medication refill, please send a message directly to your provider through the OnPatient portal at least 3–5 business days before you run out of medication. Because the portal is connected to your medical chart, it is the most streamlined and efficient way to reach your provider. You may also message our office through Weave at 469.777.4691. Please note that refills may be delayed if you are due for a follow-up appointment, have not been seen as recommended, or are not current with any required urine drug screenings. Keeping your appointments and screenings up to date will help ensure a smoother refill process.
How to Request Medical Records ?
To request a copy of your medical records, please call our office and ask a staff member to send you a Release of Information (ROI) form. If you are requesting the records for yourself, the ROI must list you as the authorized recipient, as the completed form serves as your official records request. If the records are being sent to another provider, facility, attorney, or organization, please include the full name of the receiving entity, along with the appropriate phone and fax numbers. Please note that paper copies of medical records are subject to a $25 fee.
